RESUMEN

This study assessed the mechanical properties and surface characteristics of dental prosthetic acrylic resin fabricated by 3D printing, comparing it with subtractive, pressing, and molding techniques. Bar-shaped specimens (N= 90; 65 × 10 × 3.3 mm; ISO:207951) were prepared and assigned into six groups: PRINT (3D printing vis stereolithography with PriZma 3D Bio Denture, Makertech Labs); SUB (subtractive manufacturing with Vipiblock Trilux, Vipi); PRESS Base (pressing using muffle with Thermo Vipi Wave, Vipi for base); PRESS Tooth (pressing with Onda-cryl, Clássico for tooth); MOLD Base (molding using addition silicone with Vipi Flash, Vipi for base); and MOLD Tooth (molding with Dencor, Clássico for tooth). Monotonic flexural strength (FS) and elastic modulus (E) were measured using a three-point bending approach (n= 5) on a universal testing machine at a crosshead speed of 5 mm/min. Fatigue testing (n= 10) followed similar geometry and settings, with a frequency of 2 Hz, initial stress level at 20 MPa, and stress increments of 5 MPa every 2,500 cycles. Surface roughness (n= 10) was assessed through profilometry, and fractographic and topographic analyses were conducted. Statistical analyses included One-Way ANOVA for monotonic FS, roughness, and E, along with Kaplan-Meier with Mantel-Cox post-hoc and Weibull analysis for fatigue strength. PRINT showed lower monotonic FS than the SUB and PRESS Tooth but comparable fatigue strength to these groups and superior to PRESS Base and MOLD (Base and Tooth) groups. All groups had similar Weibull moduli. Surface roughness of the PRINT group was comparable to most techniques but higher than the PRESS Tooth group. Fractographic analysis revealed fractures originating from surface defects under tensile stress, with SEM showing scratch patterns in all groups except PRINT, which had a more uniform surface. Despite its lower monotonic strength, 3D printed resin demonstrated comparable fatigue strength to subtractive and pressing methods and similar surface roughness to most methods, indicating its potential as a viable option for dental prosthesis.

The aim of this study was to evaluate the surface properties and fatigue mechanical behavior of an advanced lithium disilicate ceramic in comparison to lithium disilicate and zirconia. First, discs (n = 15, diameter = 13.5 mm and thickness = 1.2 mm) were made from the following materials: 4Y-PSZ - 4% mol yttria-stabilized zirconia (IPS e.max ZirCAD A2); LD - lithium disilicate (IPS e.max CAD); ALD - advanced lithium disilicate (CEREC Tessera). The specimens were crystalized/sintered and subsequently analyzed by a rugosimeter (Mitutoyo SJ-410) to determine surface roughness (parameters Ra and Rz). Specimens were subjected to biaxial flexural fatigue testing using the step-test method (20 Hz; 10,000 cycles per step; initial stress of 200 MPa; and step size of 25 MPa) until specimen fracture. Statistical analyses included Shapiro-Wilk, Kruskal-Wallis, and post-hoc tests for roughness data, while survival analysis (Kaplan-Meier and Mantel-Cox) and reliability analysis (Weibull modulus) were applied to flexural fatigue strength data. Hardness (Vickers) results were submitted to analysis of variance (1-way ANOVA) and Tukey's test. Zirconia (4Y-PSZ) showed higher FFS, CFF (467 MPa and 115216 cycles) and survival compared to the other materials. ALD had the lowest FFS, CFF (215 MPa and 11,908 cycles) and survival. ALD showed lower Weibull modulus (m = 6.63 for FFS; m = 1.27 for CFF) than LD for FFS (m = 17.33), and lower than LD (m = 4.64) and 4Y-PSZ (m = 6.69) for CFF. ALD showed the lowest Ra (0.07 µm) and Rz (1.05 µm) values, while 4Y-PSZ (Ra = 0.22 µm; Rz = 1.91 µm) and LD (Ra = 0.21 µm; Rz = 2.17 µm) showed higher and similar values. Zirconia (4-YPSZ) was the hardest material, while lithia-based ceramics (LD and ALD) presented the lowest and similar hardness values. Fractures originated in surface defects in the tensile stress concentration region. ALD has lower flexural fatigue strength compared to the other tested materials, along with higher variability (lower structural reliability).

The aim of this in vitro study was to evaluate the surface roughness, translucency, fatigue failure load (FFL), and number of cycles for fatigue failure (CFF) of a recently released lithia-based material called advanced lithium disilicate and three other ceramics indicated for monolithic restorations. First, ALD (advanced lithium disilicate, CEREC Tessera, Dentsply Sirona), LD (lithium disilicate, IPS e. max CAD, Ivoclar), LS (lithium silicate-disilicate, Suprinity, Vita Zahnfabrik), and 4Y-PSZ (Yttria-stabilized zirconia, IPS e.max ZirCAD MT, Ivoclar) discs (n = 15, Ø = 10 mm and thickness = 1.0 mm) were fabricated from CAD/CAM blocks/discs, A2 shade. The discs were sintered/crystallized and subsequently analyzed by a rugosimeter (Mitutoyo SJ-410) to determine Ra and Rz surface roughness parameters. Next, they were evaluated to determine the translucency parameter (TP) using a bench-top spectrophotometer (SP60, EX-Rite). The discs were subsequently cemented to glass fiber epoxy resin discs, and the specimens were tested under cyclic loading (Step-test), immersed in distilled water at a frequency of 20 Hz, with an initial cyclic load of 200 N for 5,000 cycles and increments of 50 N every 10,000 cycles until failure. Fatigue failure load (FFL) and number of cycles for fatigue failure (CFF) were recorded for subsequent Kaplan Meier analysis, with post-hoc Mantel-Cox and Weibull analysis (α = 0.05). Complementary fractographic, topographic and energy dispersive spectroscopy analyses (EDS) were performed. 4Y-PSZ showed higher survival (p < 0.05), with higher FFL and CFF (1077 N; 180,333 cycles), followed by LD (980 N; 161,000 cycles), LS (937 N; 152,333 cycles) and ALD (910 N; 147,000 cycles). No differences were observed between the tested groups regarding Weibull modulus. ALD presented TP (28.14) equal to DL (28.27) and higher than LS (25.51). All lithia-based materials had higher translucency than 4Y-PSZ (TP = 8.62) (p < 0.05). ALD appears to have a similar elemental composition to LD and LSD for oxygen and silicon. ALD and LSD have a similar zirconium content. Fractures originated on the cemented surface of the ceramic discs. Lithia-based ceramics showed lower surface roughness, with ALD (Ra = 0.04 µm; Rz = 0.66 µm) showing the lowest values (p < 0.05). Despite showing lower FFL when compared to LD and 4Y-PSZ, ALD has compatible translucency and mechanical fatigue performance with its indication for fabricating monolithic, anterior and posterior adhesively cemented single-unit restorations. However, further studies are needed to substantiate its clinical performance.

STATEMENT OF PROBLEM: The color of monolithic zirconia restorations is obtained by presintering or postsintering coloring techniques. However, studies on the differences in surface characteristics and their influence on color stability are lacking. PURPOSE: The purpose of this in vitro study was to evaluate the influence of shading and staining techniques for a zirconia ceramic on the surface characteristics and colorimetric parameters (color difference, translucency, and whiteness index) after exposure to coffee or red wine and then polishing. MATERIAL AND METHODS: Ceramic disks (N=30; Ø10×1mm) were allocated into 3 groups: preshaded-shaded by the manufacturer (IPS e.max ZirCAD MT, shade A2); manually shaded-unshaded zirconia (IPS e.max ZirCAD MT, bleaching shade-BL) colored by the brushing technique, before sintering; stained-unshaded zirconia (IPS e.max ZirCAD MT BL) colored by the staining technique, after sintering. Spectrophotometric color assessments ensured the same initial perceived color (Vita Classical A2) for specimens included in the study (∆E00<1.77 acceptability threshold). Surface characteristics were assessed by scanning electron microscopy and atomic force microscopy. The specimens were immersed in coffee (n=5) or red wine (n=5) for 12 and 24 days and subsequently polished. The data were statistically and descriptively analyzed for color differences (∆E00), translucency parameters (TP00), and whiteness indexes for dentistry (WID), considering acceptability and perceptibility thresholds. RESULTS: The shaded groups found an irregular ceramic surface with uniformly sized zirconia crystals. The stained group found a glass-covered smoother surface. Significant alterations in color parameters (∆E00, TP00, WID) were observed with immersion in pigmenting beverages (P<.001) both after 12 days, and after 24 days. The shaded specimens had greater color alterations after immersion but benefited from the polishing procedure, which reduced color differences below an acceptable threshold in comparison with the baseline. Stained specimens had lower color alterations after immersion, but the polishing protocol was detrimental as it whitened the ceramic by subsurface exposure. CONCLUSIONS: The coloring technique influences the surface characteristics of zirconia ceramic and also the color parameters after exposure to colored beverages and polishing.

STATEMENT OF PROBLEM: New computer-aided design and computer-aided manufacturing (CAD-CAM) lithium disilicate glass-ceramics have been marketed. However, information concerning their biomechanical behavior is lacking. PURPOSE: The purpose of this in vitro study was to compare the fit and fatigue behavior of two recently introduced CAD-CAM lithium disilicate materials with the standard IPS e.max CAD ceramic and to investigate the effect of the thermal treatment for crystallization on crown fit. MATERIAL AND METHODS: Monolithic crowns (n=15) were milled from 3 CAD-CAM lithium disilicates: IPS e.max CAD (Ivoclar AG), Rosetta SM (Hass), and T-lithium (Shenzhen Upcera Dental Technology). Marginal and internal fit were evaluated using the replica technique before and after crystallization, and the fatigue behavior of the luted crowns was evaluated by the step-stress method. One-way ANOVA and the Tukey test were used to compare fit among the materials. Fatigue failure load was evaluated by the Kaplan-Meier and Mantel-Cox tests. The effect of crystallization on fit was evaluated with the paired t test (α=.05). RESULTS: Marginal fit was different between IPS e.max CAD (74 µm) and Rosetta SM (63 µm) (P=.02). T-lithium was similar to the other ceramics (68 µm) (P>.05). Occlusal internal space was similar among all materials (P=.69). Fatigue failure loads of Rosetta SM (1160 N) and T-lithium (1063 N) were similar to IPS e.max CAD (1082 N) (P>.05). The fatigue failure load of Rosetta SM was higher than that of T-lithium (P=.04). Crystallization reduced the axial internal space of all materials (P<.05) without significantly affecting marginal fit (P>.05). CONCLUSIONS: The fit and fatigue behavior of Rosetta SM and T-lithium were similar to that of IPS e.max CAD. Crystallization reduced the internal space of the crowns.

This study evaluated the efficacy of fluoride gel in arresting active non-cavitated caries lesions in permanent teeth. This randomized, triple-blind, placebo-controlled clinical trial randomized 100 schoolchildren aged 10.7 ± 2.2 years to test treatment (1.23% acidulated phosphate fluoride [APF] gel) or control treatment (placebo gel) for 4-6 applications at weekly intervals. Data collection included the visible plaque index, gingival bleeding index, visible plaque accumulation on the occlusal surfaces, eruption stage, and dental caries. The association between group and lesion arrestment was assessed using logistic regression, and estimates were adjusted for plaque accumulation over the lesion at baseline, surface type, and tooth type. Models were fitted using generalized estimating equations for accounting for the clustering of data (i.e., the same individual contributed > 1 lesion). Ninety-eight children completed the study (48 fluoride and 50 placebo). When all dental surfaces were analyzed, the likelihood of lesion arrestment was similar between both groups (p > 0.05). A secondary analysis including only the occlusal lesions in molars showed that for teeth under eruption, lesions receiving the 1.23% APF gel were about 3-fold more likely to become arrested than lesions receiving the placebo gel (OR = 2.85; 95%CI = 1.23-6.61; p = 0.01). No significant difference was detected for molars with complete eruption (p > 0.05). The benefit of fluoride gel for arresting non-cavitated caries lesions could not be identified by clinical assessment in this short-term trial. Notwithstanding, when the cariogenic challenge was greater (as on the occlusal surfaces of erupting molars), 1.23% APF gel treatment was an important tool for caries control.

OBJECTIVE: To characterize the effect of pigmentation techniques on the fatigue behavior of a 4YSZ ceramic adhesively luted onto dentin analog (glass fiber-reinforced epoxy resin). METHODS: 4YSZ ceramic discs (Ø= 10 mm, 1 mm thickness) were allocated into 5 groups: Ctrl - unshaded (IPS e.max ZirCAD MT BL); Manuf - shaded by the manufacturer (IPS e.max ZirCAD MT A2); Brush - unshaded pigmented, pre-sintering, using a brush; Stain - unshaded pigmented, post-sintering, during glaze application; Brush+Stain - combination of both techniques. Color assessments ensured the same perceived color (Vita Classical A2). Dentin analog discs (Ø= 10 mm, 2.5 mm thickness) were obtained, paired with the 4YSZ discs, and adhesively luted using a resin cement. Cyclic fatigue testing (n = 15) was run (20 Hz; 10,000 cycles/step, initial load 200 N; step-size 100 N up to 700 N; then 50 N until specimen failure). Fractographic, roughness and topography analyses were performed. RESULTS: No statistical (p > 0.05) detrimental influence were observed for fatigue outcomes (Ctrl = Manuf = Brush = Stain = Brush+Stain). However, the Brush+Stain technique induced inferior statistical (p < 0.05) fatigue performance compared to Stain. All failures were radial cracks with origin at the ceramic intaglio surface. The staining technique triggered statistically higher roughness (p < 0.05). Uniformly sized zirconia crystals were seen in the Ctrl, Manuf and Brush groups, and a smooth vitreous surface with encrusted pigments in the Stain and Brush+Stain groups. SIGNIFICANCE: Pigmentation techniques (Vita Classical A2 shade) have no detrimental effect on the mechanical fatigue properties of 4YSZ ceramic adhesively luted onto dentin analog.

This study aims to characterize the effect of pre- and/or post-sintering pigmentation techniques on the fatigue behavior of a 4YSZ ceramic (4 mol% yttrium stabilized zirconia). First, 4YSZ ceramic discs (IPS e.max ZirCAD, 15 mm diameter, and 1.2 mm thickness) were obtained and allocated according to the 'pigmentation technique' factor into 5 groups: Ctrl - unshaded ceramic disc (IPS e.max ZirCAD BL); Manuf - ceramic discs shaded by the manufacturer at block formation (IPS e.max ZirCAD MT A2); Brush - unshaded ceramic disc pigmented in the pre-sintered stage using a brush; Stain - unshaded ceramic disc pigmented in the post-sintering stage using a stain glaze (shade A2); Brush + Stain - combination of Brush and Stain techniques. The specimens were sintered and analyzed by a spectrophotometer (VITA Easyshade) for color differences (ΔE00 calculated according to CIEDE, 2000) to ensure that they present the same perceived color (ΔE00 ≤ 1.77, acceptability threshold). The specimens (n = 15) were submitted to biaxial flexural fatigue testing using a cyclic fatigue method (frequency of 20 Hz; 10,000 cycles per step; initial stress of 200 MPa; and step-size of 25 MPa) until specimen fracture. Fatigue strength (FS) and number of cycles until failure (CFF) were recorded with statistical purposes. Fractographic, complementary topography, elemental and roughness analyses were performed. The Ctrl shown the highest FS, CFF, and survival rates compared to the other conditions (p < 0.05), being only statistically similar to Brush. Stain showed the lowest FS, CFF and survival rates (p < 0.05). The Manuf and Brush + Stain shown intermediary performance. In regards of Weibull moduli, Brush shown the lowest values (lowest structural reliability), for both FS and CFF, being only statistically similar to Manuf. Topography analysis shown a uniform size of zirconia crystals in Ctrl, Manuf and Brush. In addition, a smooth glassy surface with some spots of impregnated pigment was observed in the groups where glaze was applied (Stain and Brush + Stain). This finding was corroborated by elemental analysis. Despite that, Stain and Brush + Stain presented the roughest surface (p < 0.05). In summary, the pigmentation techniques used to provide a Vita classical A2 color shade have a detrimental effect on the mechanical fatigue properties of a 4YSZ ceramic, except for the technique performed with the brush in the block pre-sintering stage. Despite this, the brush technique presents great variability in mechanical performance, resulting in lower structural reliability.

This study aims to characterize the effect of shading techniques on the fatigue behavior of a 4YSZ ceramic (4 mol% yttrium stabilized zirconia) adhesively bonded to a dentin analogue (fiber-reinforced epoxy resin). 4YSZ ceramic discs (IPS e.max ZirCAD, Ø = 10 mm and 1 mm of thickness) were allocated according to the factor 'shading technique' into 4 groups: Brush- unshaded ceramic disc (IPS e.max ZirCAD BL) pigmented at the pre-sintered stage with pigment solution applied manually using a round liner brush; Immersion- unshaded ceramic disc pigmented through immersion in the solution for 1 s on only one side of the ceramic disc; Manufacturer- specimens already shaded by the manufacturer (IPS e.max ZirCAD MT A2 - Manufacturer group); Control- a control condition with absence of pigment (i.e. non-pigmented specimens). The specimens were sintered and a spectrophotometer (SP60, EX- Rite) was used to ensure that the same perceived color (i.e. pigment saturation) was achieved in the different shading strategies (Manufacturer, Brush or Immersion groups). To do so, the color differences (ΔE00) were calculated using the CIEDE 2000 equation; and an ΔE00 of up to 1.77 was considered as an acceptability threshold. Dentin analogue discs were obtained (Ø = 10 mm and 2.5 mm of thickness) and randomly allocated into pairs with the 4YSZ ceramic discs. Next, the pairs were adhesively bonded using a resin cement (Multilink N). The bonded assemblies (n = 15) were tested for fatigue using the step-stress test method (frequency of 20 Hz; 10,000 cycles per step, initial load 200 N; step-size of 100 N, up to 700 N; and after, step-size of 50 N, until specimen failure/fracture or radial cracks). Fatigue failure load (FFL) and number of cycles for failure (CFF) were recorded for statistical analysis. Fractographic features were accessed, and complementary roughness, topography, grain size and phase content analyses were performed. No statistical differences were observed in the fatigue behavior among the non-shaded condition (Control group - 880 N) and the shaded specimens (Manufacturer - 887 N, Brush - 820 N, and Immersion - 850 N groups; p > 0.05). However, the use of a brush shading technique induced slightly inferior fatigue mechanical behavior of the restorative set compared to the specimens already shaded by the manufacturer (p = 0.027). No differences in Weibull modulus were observed among the tested groups. The specimens pigmented by the brush technique demonstrated a rougher surface, with statistically higher Rz values, in addition to a larger grain size in comparison to all other conditions (p< 0.05). No m-phase content was identified (only t and c phases were detected). Thus, the shading techniques used to provide a Vita classic A2 shade does not negatively affect the mechanical fatigue properties of a bonded 4YSZ ceramic. However, the brush technique has detrimental effect on the fatigue behavior compared to when the ceramic was already provided in a shaded format by its manufacturer.
